INTRODUCTION

Healthcare sector is growing in a rapid pace globally and it is one of the largest sectors in India. Healthcare constitutes of Hospitals, Medical devices, Pharmaceutics, Telemedicine, Medical tourism, Healthcare IT, Health Insurance and Public Health. The sector is in rising pace as its market is expected to reach 322 billion dollars by 2022 due to rise in income, health awareness, life style diseases and increasing market growth of Insurance industry. The Industry has both public and private players who contribute to the rising stage of health care. The private players are those who cater the needs of secondary and tertiary care to the public. The Government i.e public healthcare system focuses on rendering basic health facilities in the form of public health centres (PHC’s).

India’s public health systems is lacking behind the growth of the healthcare industry. The share of the central government has been steadily declining over the past few years on Public expenditure towards health. To match with this context Central government has initiated the launch of Ayushman Bharat, meaning Long Life India. This initiative will cater insurance needs for nearly 500 million people of India. This sounds to be great initiative that’s why it’s been called as the “world’s biggest healthcare programme”

The Government initiatives to promote public health are:

  1. To increase healthcare spending by 3 percent of GDP in 2022
  2. Government of India established AIIMS at Manethi on February 2019 to cater the healthcare needs in Haryana at a cost of Rs. 1299 crore.
  3. Union cabinet approved to set up a National Nutrition Mission with a budget of Rs. 9,046 crore in India to monitor the nutrition related interventions in the country.
  4. Government of India has launched Mission Indradhanush to improve the coverage of immunisation in the country.
  5. Till July 2019 nearly 125.7million families were enrolled in Pradhan Mantri Jan Arogya Yojna
  6. The number of medical colleges built in India is increased to 528 in February 2019

Still in India we need to develop in many areas of Public health like sanitation, hygiene, biomedical waste management and so on. The Swachh Bharath mission or Clean India movement has contributed to some extent by reducing spread of communicable diseases in India. Though we find many steps to improve the public health still we find it difficult in fighting against new communicable diseases that has emerged naturally such as like latest pandemic outbreak COVID 19. We can find everywhere it is lock down and the spread of the virus is not yet controlled. This shows where we lack and where we need to concentrate. India has taken very rapid measure to control the pandemic and still we are fighting against it.

Road ahead:

In the next ten years a lot to be changed in India. The public should trust the public health systems in India and join with Government to support the health care programmes. All government PHC’s should be maintained properly so that rural and urban people can trust on the PHC’s. In urban areas people are afraid to go to government hospitals due to poor sanitation and hygiene problems. Now the government can do a clear roadmap to enhance budgetary spending on healthcare must be increased to 5%-7% of GDP. It is not just the public healthcare system to be strengthened but to strengthen the link between health insurance, healthcare providers and the public also. Let’s hope for better India with better health.

MEDIA ON OVER SHARE:

The pandemic has boosted digital media consumption across all platforms, from online gaming, movie streaming, forums and chat rooms.

Some of us may feel overwhelmed and have the urge to form a “stay-at-home journal” and post excessively. But what if increasing your social media exposure does more harm than good?

Bars, restaurants and shopping malls are now closed. However we’re fortunate to live in a time where technological advancements allow us to hear and see each other over great distances. Forums and instant communication apps help us stay in touch with family and coworkers, and sometimes help us make new friends.

When loneliness kicks in, we become more vulnerable. If you’re confident that nothing bad can happen in an online dating app, you’re wrong. Anyone can use a different identity in the virtual world. The flexibility and anonymity people enjoy online is a double-edged sword, as you can’t really identify truth from fiction with any certainty.

In the context of the Covid-19 outbreak and the isolation that follows, people are more likely to access online portals designed to help users date, share their story or have a good laugh – we all need one.

ONLINE DECEPTION:

Online deception has crept into every known part of the net, and creating a fake account is simply the primary domino. Some might even play the victim card, claiming they were attacked or need medical aid  or their house was vandalized by robbers or they were laid off to do so.

 They will play on your empathy and may ask you to send them money. If you deny their request, things can get ugly. At best, they’ll simply block you and head on to the subsequent victim. In some cases, however, the scammers will even attempt to threaten you by exposing any secrets you would possibly have willingly shared with them.

While there is an outburst of do’s and don’ts on how to manage and curtail the virus or how fast it’s spreading into countries and cities as we speak,  Here are some tips on social distancing  during the lockdown period

The world of work prosper on planning, however, this unpredictability has disrupted our routine work  corporate, big or small around the world are forced to shift to remote work in response to the spread of COVID -19. The impact from the health, safety, finance and economy perspective are unimaginable.

Anxiety and fear seem to be growing and casting a cloud of uncertainty around people. The continuous media cycle and deluge of WhatsApp messages surrounding the outbreak, has frightened us more. We have all been asked to practice social distancing and working from home (WFH).

Here are some of the tips on social distancing and work from home

  • Dedicated Office Space

Not everyone has a separate room that can be converted into an office. So, it could be your study table or the dining table in the living room. Do not sit in your bedroom. If you have no choice, draw the curtains and let natural light filter in.

  • Ground Rules

Ensure you have a login time and a log out time with your organization. When you are at your workstation, focus on work. Make it look office-like with peripherals and stationery.

Dress to Impress

 DON’T wear comfortable semi-formal clothes. Wear a formal dress wherein it will put you in work-mode.

Be Seen and Heard

Since most of the work require video calls and conference calls, make your presence felt by being present, physically and by speaking up, else you’ll be lost in the sea of Zoom videos and chaos of conference calls.

Eat well and Right.

When you are at home and busy with work, the temptation to skip or eat too much is a possibility. Plan your menu to provide you adequate nutrition, needed. Fortify yourself with regular water breaks.

MOVE.

 Keep moving every 30 minutes. Do five neck rolls, ten squats, ten high jumps and five body rotations to keep yourself  healthy.Many organizations are encouraging clients to stay fit at home and posting instructional videos online, too.

Call Friends and Colleagues.

While you may not be able to visit people in person, that does not mean that you cannot connect. The ideal way to overcome this period of isolation is to reach out to a friend, family or people you trust and share your concerns and feelings. Discuss lighter things like movies, music, books to keep yourself  positive.

Learn Something New.

Make time to focus on the things you wanted to learn but didn’t have the time for. Sign up for an online course – could it be a value added course, photoshop, a language, music or watch one TEDx or hear one podcast that adds value to you, personally or professionally.

Restrict Media Consumption.

At the moment, the media is on hyper-drive with news of gloom and despair. Go to news channels that are trustworthy. Restrict friends posts if they are constantly pushing messages that creates stress.

 Indulge in Self-care.

Avoid obsess about the coronavirus and include practices that promote health and positivity like yoga, meditation etc. Sleep is important during this time and practicing sleep hygiene ensures you get sufficient rest.

Power of Music.

Music has a therapeutic effect on your mood and emotions elevating joy and positivity and reducing any influence of pain and anxiety. As it can normalize your blood pressure, steadies the heart rate, and relieve your stress, so do not underestimate the power of music.

While none of us are clear when this pandemic will subside and how long we need to practice social distancing, it is not the time to allow fear to dictate your life. Let social-distancing not isolate you. Let working from home not break the social bond of teamwork. Let this time of crisis not fade out our happiness. Look for possibilities that take you ahead. Indian Economy- Demonetisation and Corona – A Comparative study.

Impact of Demonetisation:

            In November 2016 the Government took a bold step to ban 1000 and 500 Rupees note completely. This big step has taken to destroy fake currencies, resist tax evasion, extinguish the black money and terrorist financing and to look forwards towards cashless Digital India. This decision was taken for the welfare of the Country. Due to this decision there were many controversies and many faced many financial problems. There are few impacts because of the demonetisation are as follows.

1.MCRL Rates (Marginal cost Landing Rates) has increased in Several Commercial and Private Banks.

  1. Nifty fell apporx. 6.3% and BSE fell 5.9%. (just in month period)
  2. The targeted economic growth rate about 7.5% which was unable to achieve.
  3. India’s 45% of GDP are given by IT industries they also have been affected.
  4. The monthly average sales of real estates have reduced from 49% to 40%.
  5. FMCG industry has gone by 1% to 1.5%, the purchase of FMCG has gone down to 6.5%.

Note: The demonetization is not new to us, 10,000 rupees note where printed in 1938 and again in 1954. These notes where demonetized in 1946 and again in 1976. But this did not have any impact in the Economy because only 5% of the public where having this Currency.

            This impact was during demonetization, in which the economy had a short fall. Slowly we were able to get back to growth rate of our economy . But now we are facing an Economic Crisis which is affecting all the countries including our Country. This is because of World Health Threat, the virus out-break during Dec 2019 & Jan 2020 in Wuhan, China Novel Corona Virus.

Impact of Corona in Indian Economy:

Novel Corona Virus a new Threat to Global Man Kind. This was discovered in the recent times and the out break of this infection is more than what we have seen during the outbreak of SARS 2015. Due to this COVID-19 the world economy had short fall. Many Countries like Italy, China etc., have been suffering both physically and Economically. Even our country also facing the Economic crisis due to the out break of Novel Corona Virus.

            Indian’s Economy could take a hit of up to half a percent in this FY21 said by the Indian Economic Analysists.

  • There will be a hit of 0.3% to 0.5% in GDP in the next fiscal year. In the first 2 quarter it was 4%-4.5%. Maybe this may drop to 3.9% to 4% in GDP.
  • India cannot attain the planned 5% growth rate in the current fiscal year.
  • RBI expect the current account deficits would be 22-24 billion dollars.
  • Trade deficit fell to the lowest level of 9.85 billion dollars.
  • The Exports declining at the rate of 2.91% in February 2020 it was 27.65 Billion Dollars.

Let’s see what all the sectors which have been affected due to this COVID-19 outbreak.

1.Aviation:

585 international flights have been cancelled by the private passengers in March 6. On March 12th India has cancelled all the Visas which was issued earlier. India’s largest airlines has reported that they had 10%-15% of Decline.

  1. Hospitality:

            The restaurant business has fallen by 35%. The average occupancy has fallen from 70%-75% to 20%. National Restaurant Association of India has reported that restaurant’s business has been declined to 35%.  Many of the Multiplexes from major cities they have been shut down.

3.Apparels:

            Many countries have denied the products from India almost 35% of apparels are waiting for the Exports. More than 60% of the revenue of Apparels comes from Export which is almost one lakh crores.

  1. Consumer Durable and Electronics:

            More than 15% of Consumer Durables has fall in during the period of Jan-Mar 2020. India Imports more than 45% of Accessories and Spares from China which is also denied.

  1. Poultry and Seafood:

            More than 30% fall in demand of chicken in past three week. Poultry industry are forced to sell the chickens at a cost of 30rs. The stock pileup for the Poultry Industry due to the outbreak of the Virus and the Fake news which was circulated among the public is cause of the pileup. Many European countries and many other countries have denied the Export of Seafoods.

            Due the outbreak of Novel Corona Virus (COVID-19) many sectors have their decline stage and Indian Economic growth is started to fall down. Imports and Exports have stopped for the short period of time due to that Tons and Tons products have been stocked to delivered. If this situation continues many businesses is going to end up. Analysist are predicting that the situation is going to still worse in the month of April and May.

The retailers who use demand oriented pricing approach should understand the psychological implications of the retail price on customers. The stated sales goals can be achieved by estimating the quantity of goods that the customers demand for various products at different level of price. This demand oriented pricing used by the retailer’s struggles to gain sales volumes and market share.  The retailers should consider the following types of pricing

  • Price Quality Association

Most of the customers perceive a high priced product as a high quality product and a low priced product as low quality product.

  • Prestige Pricing

   Retailers using this pricing approach do not keep low value goods and services in their store as they think customer may perceive the store as low quality store.

    Clinical Decision support system is a latest trend of healthcare information. This technology provides physicians and healthcare providers to make clinical decisions. Robert Hayward defined “Clinical decision support systems  link health observations with health knowledge to influence health choices by clinicians for improved health care”. CDSS is a form of Artificial Intelligence in Medicine.

    A clinical support system is making an advice clinically to the patient based on the multiple patient data. As a health IT tool CDSS has an all over reach in the healthcare sector. It is applied in the healthcare sector to improve the quality, avoid medical errors and know about the patients’ health information well in advance. CDSS as a tool enables to fix up reminders for preventive care, give alert about the dangerous drug interactions and alert healthcare providers to initiate the test that patient has to undergo. It can also give a precautious sign if there has been any kind of improper diagnosis, missed or wrong dosage of medications.

CPOE – Computerized Physician Order Entry is one of the forms of CDSS where physicians automate the medications order. CPOE is a standardized format where physicians can enter the medications and it can be integrated with CDSS for enhancing the patient safety.

Applications of CDSS in hospitals:

  1. A hospital in Albama has decreased its sepsis mortality rates by implementing CDSS. Real time alerts where given for new diagnosis for sepsis and remainders where given to the providers to check for the worsening vital signs among the sepsis patients
  2. Mayo Clinic employs CDS that helps the nurses to deliver complete and accurate phone screenings of patients who seek appointments. The computerised software gives detail questions to the nurses so that they do not miss any information about the patient health.
  3. CDSS application with head injury patients by Yale and the Mayo Clinic gives guidelines and provides information on severity of the injury.  The application explains decision treatments to patients and reducing the number of unnecessary CT scans.

Criticisms and Solution for CDSS implementation:

            This new technology is an existing workflow of clinicians is an frustrating one.  As by AMA report clinicians spend more time on entering the data in keyboard than they spend with their patients. This makes the health care industry to change its view on the CDSS as a time-consuming process rather than time saving. Anyway, this assumption can be broken down by establishing a specific team and defining a project scope. To start with the organisation which is going to adopt CDSS technology should be ready to use it in some specific clinical departments and they should set goals and priorities for the same. The organisation should be ready to adopt any change in its work flow and this readiness will be the first step in implementing CDSS. Physicians, nurses, administrators, and other technology users should be part of the discussion while implementing CDSS and in considering CDS vendors or implementation strategies. The acceptance by the clinicians that someone discussing their patient’s diagnosis and medications will be a successful implementation of the above technology.

Collecting feedback from the users of the technology is like getting stakeholder input before going for live. The impact of patient quality care, patient safety, workflow and provider / patient satisfaction after implementation of CDSS is the feedback measure of the above technology. End user feedback can help inform future efforts and make positive changes to the process. Clinical decision support tools can improve patient safety and care quality if providers accept the addition to their workflow and don’t feel frustrated by new alerts or notifications. As CDS tool continue to grow along with machine learning and AI provider may find the future of CDS to be more intuitive and less intrusive.
